JOB TITLE: Case Manager                                                                 

PROGRAM: Intensive Case Management

FLSA STATUS:  Non-exempt

JOB SUMMARY:  Provides structured activities that promote socialization, recovery, wellness, self-advocacy, development of natural supports, and maintenance of community living skills. Intervene, stabilize and manage acute crisis situations as needed. Assists individuals in identifying strengths which aid and barriers that impede the recovery process. Ensure and provide high quality, compassionate, and ethical mental health services to improve program and organizational performance.

DUTIES AND RESPONSIBILITIES:  

  • Meet weekly contact and productivity standards
  • Possess an understanding of recovery and psychosocial rehabilitation principles as defined by the Georgia Consumer Council and psychosocial rehabilitation principles published by USPRA and must possess the skills and ability to assist other individuals in their own recovery processes.
  • Attend treatment team meetings as directed by Program Supervisor.
  • Assist with service related compliance.
  • Assist in training co-staff in areas relating to service specialties as needed.
  • Maintain confidentiality for all indirect/direct service in accordance with agency policies and HIPPA policies.
  • Other responsibilities and tasks as may be assigned.

KNOWLEDGE

  • Knowledge of necessary appropriate boundaries for working with vulnerable populations
  • Knowledge of addictive diseases and/or mental health disorders and their impact on an individual’s functioning
  • Understanding of recovery principals

SKILLS

  • Microsoft office
  • Excellent verbal and written communication skills
  • Exceptional time management skills and attention detail
  • Collaborative documentation

KEY PERFORMANCE INDICATORS (KPIs):

  • Maintain Agency performance expectations 100% of the time. Provide a minimum of 1200 billable hours per year, complete all required documentation same day services are delivered/end of shift
  • Work variable evenings and weekends
  • Will monitor failed claims and failed activities
  • Provide and document a minimum of 6 out of clinic services per month 100% of the time.
  • Clients are provided a choice of activities 100% of the time.
  • Complete and provide quality, collaborative service documentation 100% of the time.
  • 99% accurate on entering information in EMR.
  • 100% of require trainings will be completed within specified time frames.

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT

  • Exposure to hot, cold, wet, humid or windy conditions caused by weather while traveling from office to field visit.
  • Sit for long periods of time.
  • Bend or twist their body.
  • Occasional lifting up to ten lbs
  • Must be able to work in climate controlled environment
  • Requires interaction with co-workers, residents and vendors
  • Ability to climb up and down building stairs
  • Exposure unsanitary conditions at times
  • Contact with pets

MINIMUM QUALIFICATIONS:

  1. Education –  Bachelor’s in Human Services or related field
  2. Licensure/Certification – N/A
  3. Experience – Minimum two (2) years of experience providing direct care to individuals in human services and documenting electronically
  4. Other - Criminal background check clearance, official academic transcripts, valid driver’s license/ID drug screen, TB test

PREFERRED QUALIFICATIONS:

  1. Education – Master’s Degree in Human Services or related field
  2. Experience – Three (3) years of experience providing direct care to individuals in Human Services and documenting electronically
