JOB TITLE: Direct Service Professional                                               

PROGRAM: Meadows Road Group Home

FLSA STATUS: Non-exempt

JOB SUMMARY: Provide ongoing supports for participants with intellectual and/or developmental disabilities who require formal support 24 hours/day, 7 days/week. Supports include providing training and interventions with focus on one or more areas including eating and drinking, toileting, personal grooming, health care, dressing, communication, interpersonal relationships, mobility, home management, and use of leisure time.


  • Essential duties and responsibilities of the Community Residential Support Worker include, but are not limited to:
  • Provide participant-specific assistance and training in activities of daily living, such as bathing, dressing, grooming, other personal hygiene, feeding, toileting, transferring, and other similar tasks.
  • Accompany participants and facilitating participation in visits for medical care, therapies, personal shopping, recreation, and other community activities.
  • Train or assist in household care, such as meal preparation, cloths laundering, bed-making, housecleaning, shopping, simple home repair, yard care, and other similar tasks.
  • Assist with therapeutic exercises, supervising self-administration of medication and performing health maintenance Activities.
  • Implement positive behavioral support plans of participants to reduce inappropriate and/or maladaptive behaviors to acquire alternative adaptive skills and behaviors.
  • Provide transportation to all other services specified in the Individual Service Plan as needed to facilitate the individuals’ participation in personal shopping, recreation, medical appointments, and other community based activities.
  • Other responsibilities and tasks as assigned.


  • Knowledge of Georgia Department of Community Health Division of Medicaid Policies and Procedures for the Comprehensive Waiver Part III, Chapter 1900-3500: Specific Program Requirements for Community Residential Alternative Services.
  • Knowledge of Georgia Department of Behavioral Health & Developmental Disabilities Provider Manual Part II, Section 1: Community Standards for Developmental Disabilities Providers.
  • Knowledge of National Accreditation Standards for CARF
  • Knowledge of Georgia Department of Community Health regarding licensure policies and procedures.


  • Demonstrate competency utilizing and managing medical records documentation systems.
  • Drive and demonstrate transporting participants in fleet vehicles with a lift and wheelchair tie downs safely.
  • Demonstrate written and oral communication skills.
  • Demonstrate positive customer service delivery to internal and external stakeholders.
  • Demonstrate person centered individualized supports with activities.
  • Demonstrate client rights knowledge and implement when supporting participants.
  • Maintain certifications for First Aid/CPR, Medication management, Seizure management, and Transfers, Visual Impaired/Blind and Hearing Impaired/Deaf protocols.


  • Ability to drive fleet vehicle with lift and use of safety protocols.
  • Ability to provide transfers to adults in wheelchairs.
  • Ability to work directly with adults with intellectual developmental disabilities who require supports across all adult daily living skills.


  • Prior Approval-Efficiency of Services-KPI #1- 100%of referrals will ensure HRST is complete and services are authorized for Residential Category 1-4 for supports commensurate with each participants’ current level of need.
  • Maintain census of residents (4 bed group home/2 host homes) –(Efficiency)-KPI # 2- 100% Maintain program census per DBHDD contract.
  • Annual mastery of Community Residential Alternative goal(s)(Effectiveness)-KPI #3 – 90% of individuals receiving Community Residential Alternative services will demonstrate mastery of residential goals in their ISP annually.
  • Customer Satisfaction-Satisfaction-KPI #6-IDD Satisfaction Survey- 80% of the individuals reporting satisfaction with Community Residential Alternative Services.


  • Ability to lift: Up to 25 pounds
  • Ability to walk a reasonable distance when on community outings
  • Duties may require operating and/or riding in a vehicle.
  • Must be able to execute the physical demands of the position including kneeling, squatting, reaching, bending
  • Use hands, fingers and type on a keyboard
  • Noise level in the work environment is usually moderate
  • Requires interaction with co-workers, residents and vendors
  • Exposure to hot, cold, wet, humid or windy conditions caused by weather while transporting.


  1. Education – High School Diploma or GED.
  2. Licensure/Certification – NA
  3. Experience – One (1) year experience with the Developmental Disabilities population
  4. Other – Criminal background check clearance, official academic transcripts, valid driver’s license/ID, drug screen, TB test.
