JOB TITLE: Direct Service Professional

JOB SUMMARY: The RIM/ NEW START/ CRISIS RESPITE Direct Service Professional performs direct care services to promote, encourage, and ensure efficient operation of the RIM/ NEW START/ CRISIS RESPITE program, ensuring the safety and wellbeing of program participants. Seeking to contribute to the success of the RIM/ NEW START/ CRISIS RESPITE program, as well as the agency as a whole, the Direct Service Professional successfully carries out daily tasks including assisting with daily living activities, monitoring of program participants’ progress, and facilitation of life skills groups, through acquired knowledge, skills, and abilities.

*Residents of these programs are in recovery from Addictive Disease and/or Dual Diagnoses

DUTIES AND RESPONSIBILITIES:

Direct Care: (75%)

  • Assist with daily living activities, monitoring, and providing support to individuals residing in a 24/7 semi-independent residential substance abuse treatment facility to ensure the safety and wellbeing during recovery.
  • Facilitates life skills groups, including education on personal hygiene, nutrition, money management, and interpersonal skills.
  • Provides structured activities that promote socialization, recovery, wellness, self-advocacy, development of natural supports, and maintenance of community living skills.
  • Monitors completion of urinalysis drug screens, breathalyzers, and self-administration of medication in accordance with agency policies and procedures, to encourage accountability.
  • Provides transportation for individuals to appointments and activities in the community.
  • Performs light housekeeping duties, such as cleaning the program office and inspection of facilities in accordance with environment of care regulations.
  • Ensures and provides high quality, compassionate, and ethical delivery of services to improve program and organizational performance.
  • Other responsibilities and tasks as assigned.
  • Maintains confidentiality for all indirect/direct service in accordance with agency policies and HIPPA policies.
  • Intervenes, stabilizes and manages acute crisis situations as needed.
  • Provides additional coverage during increased census/acuity and emergencies.

 

Administrative: (25%)

  • Prepares and maintains records of individuals’ progress and services delivered, reporting changes in condition to manager or supervisor.
  • Enacts service related compliance and timely documentation.
  • Maintains confidentiality for all indirect/direct service in accordance with agency policies and HIPPA policies.
  • Prepares and submits required reports and documentation per agency policy.

KNOWLEDGE

  • Knowledge of necessary appropriate interpersonal boundaries for working with vulnerable populations.
  • Familiarity with addictive diseases and/or mental health disorders and their impact on an individual’s functioning.

SKILLS

  • Excellent verbal and written communication skills.
  • Exceptional time management skills and attention to detail.
  • Familiar with computer hardware and software including Microsoft Office products (Word, Excel, Outlook), e-mail, fax, and scanning.
  • Familiar with EMR documentation

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT

  • Requires interaction with co-workers, residents, vendors, and community stakeholders
  • Ability to lift: Up to 25 pounds
  • Ability to climb up and down building stairs
  • Ability to walk a reasonable distance when on community outings

 

KEY PERFORMANCE INDICATORS (KPIs):

  • Maintain Agency performance expectations 100% of the time.
  • Complete and provide quality, collaborative service documentation 100% of the time.
  • Complete all required documentation same day services are delivered/end of shift (except for evening groups in which documentation is due by 11:00 AM next business day).
  • 99% accurate on entering information in EMR.
  • 100% of required trainings will be completed within specified time frames.

MINIMUM QUALIFICATIONS:

  • Education – High School Diploma/GED required.
  • Licensure/Certification – N/A
  • Experience - Minimum One (1) year of direct care service for individuals in human services setting and experience with electronic documentation.
  • Other – Criminal background check clearance, official academic transcripts, valid driver’s license/ID, drug screen, TB test, eligible 7yr MVR.

PREFERRED QUALIFICATIONS:

  • Education – HS Diploma/GED, working towards post-secondary education.
  • Experience – Two (2) years of direct care service for individuals with addictive diseases and/or mental health and experience with electronic documentation in an EMR.
