JOB TITLE: Case Manager  (COPS)                                                                       

PROGRAM: Case Management

FLSA STATUS: Non-exempt

JOB SUMMARY:  The Case Manager provides direct care services to the adult Severely-Persistent Mentally Ill population. The Case Manager’s carry a caseload of up to 50 clients that provide Case Management and PSR-I to clients in the Cobb, Douglas, and Cherokee Counties. Case Managers also provide Crisis Intervention/Emergency services as appropriate.

DUTIES AND RESPONSIBILITIES:  

DIRECT CARE                                                                                                                           

  • Teaches skills to improve day-to-day function and overall well-being                                   
  • Assist client with identifying, accessing and maintaining community resources               
  • Completes daily clinical documentation in accordance with the agency policy                  
  • Intervene, stabilize and manage acute crisis situations as needed                                     

ADMINISTRATIVE

  • Maintain clinical records in accordance with agency policies.                                                       
  • Ensures timely completion of agency required trainings (Relias, CPI, CPR/First Aid, etc…)       
  • Ensure timely completion of agency required trainings/workshops.
  • Completes Incident Reports, Staff Errors in timely manner.                                                    
  • Completes the appropriate paperwork to transition to lower/higher levels or care.         

KNOWLEDGE

  • Appropriate client/staff boundaries
  • Knowledge of understanding recovery principles

SKILLS

  • Excellent verbal and written communication skills
  • Exceptional time management skills and attention to detail
  • Familiar with computer hardware and software including: Microsoft Office Products (Word, Excel, PowerPoint, Outlook), email, fax, and scanning

ABILITIES

  • Ability to manage and prioritize multiple assignments to meet deadlines
  • Ability to provide structured activities that promote socialization, recovery, wellness, self-advocacy, development of natural supports, and maintenance of community living skills

KEY PERFORMANCE INDICATORS (KPIs):

  • Maintain agency performance expectations 100% of the time. Provide a minimum of 1200 billable hours per year, complete all required documentation same day services are delivered/end of shift
  • Work variable evenings and weekends
  • Will monitor failed claims and failed activities
  • Provide and document a minimum of 2 out of clinic services per month, per authorized service 100% of the time.
  • Complete and provide quality, collaborative service documentation 100% of the time.
  • 100% of require trainings will be completed within specified time frames.

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT

  • Exposure to hot, cold, wet, humid or windy conditions caused by weather while traveling from office to field visit.
  • Sitting for long periods of time.
  • Bend or twist body.
  • Occasional lifting up to 10 lbs
  • Must be able to work in climate controlled environment
  • Requires interaction with co-workers, residents, community partners, and vendors
  • Ability to climb up and downstairs
  • Exposure unsanitary conditions at times
  • Contact with pets

MINIMUM QUALIFICATIONS:

  1. Education: Bachelors in Human Services Field
  2. Licensure/Certification: – N/A
  3. Experience: 2-3 year’s providing direct care to individuals in the Human Services field  documenting electronically

PREFERRED QUALIFICATIONS:

  1. Education – Master’s Degree in Human Services or related field
  2. Licensure/Certification – NA
  3. Experience – 3 years of experience providing direct care to individuals in Human Services and documenting electronically
